Renew your Oregon Health Plan Benefits

Renew your Oregon Health Plan Benefits

Did you receive a letter asking you to renew your OHP benefits?

Every year, Oregon Health Plan (OHP) and Healthy Kids members must update their information to make sure they still qualify for health benefits. When it is time for you to renew your benefits, OHP will send you a letter with instructions on how to renew. If you do not receive a letter, then you do not have to renew your benefits at this time.

Here’s what you should do if you get a renewal letter:

1. Fill out the requested information for your household
2. Sign the form or application
3. Send it back in the envelope included with the letter
When you get a renewal letter, you can also renew your information over the phone by calling OHP Customer Service at 1-800-699-9075.

We’re here to help. NAYA Family Center can help you through the renewal process. Contact us at 503-288-8177 x 338 Kristen Bell or x 308 Chonitia Smith. More information about the renewal process is available at www.ohp.oregon.gov

After you submit your updated information, OHP will process your renewal. You should hear back within 45 days from the date you submitted your renewal form or application.
